Indications

Description
- Selective, reversible inhibitor of PDE5
- Increases levels of cGMP in the corpus cavernosum
- Results in smooth muscle relaxation and inflow of blood into the penile tissues
Pharmacology
- Mechanism of Action: Release of nitric oxide leads to increased levels of cGMP, producing smooth muscle relaxation and allowing inflow of blood
- Pharmacokinetics and Metabolism: Rapid absorption, hepatic metabolism, terminal half lives of about 4 hours
- Absorption and Distribution: Rapidly absorbed, maximum observed plasma concentrations reached within 30 to 120 minutes
- Metabolism and Excretion: Cleared by hepatic microsomal isoenzymes
Dosage & Administration
- Erectile dysfunction: Recommended dose is 50 mg taken approximately 1 hour before sexual activity, with possible adjustment to 100 mg or 25 mg based on toleration
- Pulmonary arterial hypertension: Recommended dose is 20 mg three times a day
Interaction
- Metabolism is mediated by CYP3A4 and CYP2C9
- Inhibitors of these isoenzymes may reduce clearance
- Inducers of these isoenzymes may increase clearance

- Hypersensitivity to any component
- Concomitant use with organic nitrates
