Indications
- Treatment of B-9 deficiency
- Treatment of megaloblastic anemia
- Treatment of anemias of nutritional origins, pregnancy, infancy, or childhood
Pharmacology
- Conversion of folic acid to tetrahydrofolic acid and methyltetrahydrofolate by dihydrofolate reductase (DHFR)
- Transportation across cells by receptor-mediated endocytosis
- Maintenance of normal erythropoiesis
- Synthesis of purine and thymidylate nucleic acids
- Interconversion of amino acids
- Methylation of tRNA
- Generation and use of formate
- Normalization of high homocysteine levels by remethylation of homocysteine to methionine via methionine synthetase using vitamin B12 as a cofactor
Dosage & Administration
- Adults:
- Initial dose of 5 mg daily for 4 months
- Maintenance dose of 5 mg every 1-7 days depending on the underlying disease
- Children:
- 500 mcg/kg daily for children up to 1 year old
- Same as adult dose for children over 1 year old

Contraindicated in patients who have shown previous intolerance to the drug

Precautions & Warnings
Caution in patients with folate dependent tumors, should not be given alone or in inadequate amounts with vitamin B12 for undiagnosed megaloblastic anemia, should not be given alone in vitamin B12 deficiency states, cobalamin absorption test recommended for elderly people before long term folate use, no harmful results found from short courses of folate
