Indications

Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Specific inhibition of the H+/K+-ATPase in the gastric parietal cell
- First single optical isomer of proton pump inhibitor
- Better acid control than racemic proton pump inhibitors
- Absorption after oral administration
- Peak plasma levels occur at approximately 1.5 hours
- Distribution - 97% bound to plasma proteins
- Metabolism by cytochrome P450 (CYP) enzyme system
- Plasma elimination half-life of approximately 1-1.5 hours
Dosage
- Oral dosage form for healing erosive esophagitis, symptomatic GERD
- Oral dosage form for Helicobacter Pylori eradication, Zollinger-Ellison Syndrome, Acid-related Dyspepsia, Duodenal & Gastric ulcers
- IV Injection or IV infusion for GERD with Erosive Esophagitis, Risk reduction of rebleeding of Gastric or Duodenal Ulcers
- Different dosages for pediatric and adult patients
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral Suspension should be taken after mixing with water
- IV Injection solution for injection is prepared by adding 0.9% Sodium Chloride
Interaction
- Extensively metabolized in the liver by CYP2C19 and CYP3A4
- No clinically relevant interactions with drugs metabolized by CYPs 1A2, 2A6, 2C9, 2D6, 2E1 and 3A4
- Potential interference with CYP2C19, the major metabolizing enzyme
- Co-administration with clarithromycin and amoxicillin increases plasma levels
Contraindications
- Contraindicated in patients with known hypersensitivity to any of the formulation

- Abdominal pain
- Constipation
- Dry mouth
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- No teratogenic effects observed in animal studies
- Excretion in milk not studied
- Breast-feeding should be discontinued if essential
Precautions & Warnings
- Symptomatic response does not preclude the presence of gastric malignancy
- Tablet or capsule should be swallowed at least one hour before meals
- Usage in patients with difficulty swallowing capsules
Use in Special Populations
- Safety and effectiveness in paediatric patients have not been established
- No overall differences in safety and efficacy in geriatric patients
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- No alteration in pharmacokinetics in patients with renal impairment
Overdose Effects
- Lethal to rats at high doses
- Symptoms include reduced motor activity, respiratory changes, ataxia, and convulsions
- No specific antidote known

- Store at a temperature not exceeding 30°C in a dry place
- Protect from light and moisture
- Keep out of reach of children
