Pharmacology
Decreases inflammation by inhibiting leukocyte migration and capillary permeability, and suppresses normal immune response
Dosage & Administration
- Adjust dosage based on severity and patient response
- Oral dosages vary from 1 mg to 10 mg for adults
- Higher dosages may be temporarily required in some cases
- Intravenous, intramuscular, or local injection administration
- Dosage varies with severity of condition
Interaction
- Enhanced potassium loss with diuretics and/or cardiac glycosides
- Impaired glucose tolerance with antidiabetic drugs
- Increased risk of gastrointestinal ulceration with non-steroidal anti-inflammatory drugs
- Alteration of need for oral anti-coagulants
- Reduced efficacy when used with liver enzyme inducing drugs
- Need for caution when using with salicylates
- Impaired gastrointestinal absorption when used with antacids
Contraindications
- Gastric and duodenal ulcers
- Systemic and ophthalmic fungal infections
- Viral infections
- Glaucoma
- Hypersensitivity to corticosteroids
Side Effects
- Endocrine and metabolic disturbances: Cushing-like syndrome, hirsutism, menstrual irregularities
- Fluid and electrolyte disturbances: Sodium and fluid retention, hypertension, potassium loss
- Musculo-skeletal effects: Myopathy, osteoporosis
- Gastro-intestinal effects: Ulceration, perforation, haemorrhage
- Immunosuppressive effects: Increased susceptibility to infections
Pregnancy & Lactation
Category C; should be used during pregnancy if potential benefit justifies risk, appear in breast milk
Precautions & Warnings
- Use lowest possible dose
- Caution in patients with heart failure, hypertension, or renal insufficiency
- Minimize secondary adrenocortical insufficiency with gradual dose reduction
- Use with caution in patients at risk of osteoporosis
- Acute myopathy observed with high doses, especially in myasthenia gravis patients
- Psychic derangements may appear
Use in Special Populations
Titrate pediatric patients to lowest effective dose to minimize potential growth effects
Overdose Effects
Treatment is by supportive and symptomatic therapy
