Pharmacology

Pregabalin is a structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA). It binds with high affinity to the alpha2-delta site in central nervous system tissues, contributing to its anti-nociceptive and antiseizure effects.

Dosage

  • Maximum recommended dose of Pregabalin for diabetic peripheral neuropathy is 100 mg three times a day
  • For postherpetic neuralgia, the recommended dose ranges from 75 to 300 mg/day based on efficacy and tolerability
  • For fibromyalgia, the recommended dose is 300 to 450 mg/day
  • For neuropathic pain associated with spinal cord injury, the recommended dose range is 150 to 600 mg/day

Administration

  • Route of administration: Oral route, may be taken with or without food
  • Pregabalin CR tablet administration: After an evening meal, should be swallowed whole and not split, crushed or chewed

Interaction

  • Drug interaction with medication: Unlikely to be involved in significant pharmacokinetic drug interactions
  • Drug interaction with food and others: Not applicable

Contraindications

Contraindicated in patients with known hypersensitivity to Pregabalin or any of its components

Side Effects

Common side effects include dizziness, somnolence, dry mouth, edema, blurred vision, weight gain, and difficulty with concentration/attention

Pregnancy & Lactation

No adequate and well-controlled studies with pregabalin in pregnant women, small amounts detected in lactating women's milk, breastfeeding not recommended

Precautions & Warnings

  • Angioedema can occur and may be associated with life-threatening respiratory compromise
  • Discontinue immediately in cases of hypersensitivity reactions and certain adverse effects
  • Increase the risk of suicidal thoughts or behavior
  • Respiratory depression may occur when used with concomitant CNS depressants

Use in Special Populations

  • Use in children and adolescents: Safety and effectiveness not established for various conditions

Overdose Effects

Signs and symptoms include reduced consciousness, depression/anxiety, confusional state, agitation, restlessness, seizures, and heart block. No specific antidote.
