Indications

  • Active duodenal ulcer
  • Active benign gastric ulcer
  • Symptomatic erosive or ulcerative gastro-esophageal reflux disease (GERD)
  • Gastro-esophageal Reflux Disease Long-term Management (GERD Maintenance)
  • Symptomatic treatment of moderate to very severe gastro-esophageal reflux disease (symptomatic GERD)
  • Zollinger-Ellison Syndrome
  • In combination with appropriate antibacterial therapeutic regimens for the eradication of Helicobacter pylori in patients with peptic ulcer disease

Pharmacology

Rabeprazole suppresses gastric acid secretion by inhibiting the gastric H+/K+-ATPase at the secretory surface of the gastric parietal cell.

Dosage

  • 20 mg once daily in the morning for active duodenal ulcer and active benign gastric ulcer
  • 20 mg once daily for four to eight weeks for erosive or ulcerative gastro-esophageal reflux disease (GERD)
  • Maintenance dose of 20 mg or 10 mg once daily for long-term management of GERD
  • 10 mg once daily for symptomatic treatment of moderate to very severe GERD
  • 5 mg once daily for pediatric patients 1 to 11 years of age (less than 15 kg)
  • 10 mg once daily for pediatric patients 1 to 11 years of age (15 kg or more)
  • 60 mg once a day for Zollinger-Ellison Syndrome
  • Eradication therapy with rabeprazole sodium 20 mg twice daily, clarithromycin 500 mg twice daily and amoxicillin 1g twice daily for H. pylori infection

Administration

Rabeprazole tablets should be taken in the morning, before eating, and should be swallowed whole without chewing or crushing.

Interaction

  • Potential interaction with compounds whose absorption is pH dependent
  • Significant decrease in antifungal plasma levels when co-administered with ketoconazole or itraconazole
  • No interaction with liquid antacids observed
  • Not recommended for co-administration with atazanavir due to pH-dependent absorption of atazanavir

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Contraindicated in pregnancy and during breastfeeding

Precautions & Warnings

  • Symptomatic response does not exclude the possibility of malignancy
  • Regular surveillance for long-term treatment
  • Risk of hip, wrist, and spine fracture especially in the elderly or in presence of other risk factors
  • Risk of cross-hypersensitivity reactions with other proton pump inhibitors or substituted benzimidazoles
  • Potential for blood dyscrasias, hepatic enzyme abnormalities, hypomagnesaemia, and interference with laboratory tests

Use in Special Populations

  • No dosage adjustment for patients with renal or hepatic impairment
  • Not recommended for use in children due to lack of safety and efficacy data

Overdose Effects

Effects are generally minimal and reversible without further medical intervention. No specific antidote is known.
