Indications
- Adjunct to diet and exercise for noninsulin dependent diabetes mellitus (Type II)
- May be used in combination with metformin and insulin for hyperglycemia control
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves beta cell sensitivity to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Hypoglycemic action lasts for 24 hours
Dosage
- Initial dose is 1 mg once daily
- Dose titration based on regular blood sugar monitoring
- Usual dose range for well-controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
- Tablet should be swallowed with sufficient liquid

- Tablet must be swallowed with sufficient liquid
Interaction
- Potentiating drugs: ACE inhibitors, aminosalicylic acid, anabolic steroids, etc.
- Attenuating drugs: Acetazolamide, barbiturates, corticosteroids, etc.
- Alcohol intake may potentiate or attenuate the activity in an unpredictable fashion
Contraindications
- Not suitable for type I diabetes mellitus, diabetic ketoacidosis, or diabetic coma
- Hypersensitivity to glimepiride, other sulfonylureas, sulfonamides
- Severe hepatic dysfunction, severe impairment of renal function, dialysis patients

Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary for pregnant patients
- Harmful during breastfeeding
- Changeover or discontinuation of breastfeeding necessary for lactating women
Precautions & Warnings
- Increased risk of hypoglycemia in initial weeks of treatment
- Hypoglycemia can be promptly controlled by immediate intake of carbohydrates
Overdose Effects
- Overdosage can produce hypoglycemia
- Mild symptoms should be treated aggressively with oral glucose
- Severe reactions require immediate hospitalization and intravenous glucose
- Patients should be closely monitored for at least 24 to 48 hours
