Indications
- Adjunct to diet and exercise for noninsulin dependent diabetes mellitus (NIDDM)
- Combination use with metformin
- Combination use with insulin
- Increased risk of hypoglycaemia with insulin combination

Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves beta cell sensitivity to glucose
- Reduces hepatic glucose production
- Enhances peripheral tissue sensitivity to insulin
- Sustained hypoglycaemic action for 24 hours
Dosage
- Dependent on desired blood sugar level
- Initial and maintenance doses based on regular glucose monitoring
- Gradual dose titration
- Usual initial dose of 1 mg once daily
- Dose range of 1 to 4 mg daily for well controlled diabetes
- Timing and distribution of doses decided by physician
Administration
- Tablet to be swallowed with sufficient liquid
Interaction
- Potentiating drugs: ACE inhibitors, anabolic steroids, azapropazone, fluoxetine, guanethidine, sulfonamide antibiotics
- Attenuating drugs: Calcium channel blockers, corticosteroids, diuretics, nicotinic acid, phenothiazines
- Masking effects by beta-receptor blockers, clonidine, guanethidine, reserpine
- Unpredictable effect with alcohol intake
Contraindications
- Insulin dependent diabetes mellitus
- Diabetic ketoacidosis
- Diabetic coma
- Hypersensitivity to sulfonylureas or sulfonamides
- Severe hepatic dysfunction
- Severe impairment of renal function
- Dialysis patients

Pregnancy & Lactation

- Prompt control of hypoglycaemia with immediate carbohydrate intake
Overdose Effects
- Hyoglycaemia from sulfonylurea overdosage
- Mild symptoms treated with oral glucose and adjustments in dosage or meal patterns
- Severe reactions require immediate hospitalization and intravenous glucose
- Close monitoring for recurrence
