Indications
- Adjunct to diet and exercise for lowering blood glucose in NIDDM
- Used in combination with metformin or insulin
- May increase potential for hypoglycaemia
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin from pancreatic beta cells
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Initial dose 1mg, can be increased based on glucose monitoring
- Dose range 1-4mg daily for well-controlled diabetes
- Single daily dose before substantial breakfast or first main meal
Administration
- Must be swallowed with sufficient liquid
Interaction
- Potentiating drugs: ACE inhibitors, MAO-inhibitors, salicylates, sulphonamide antibiotics
- Attenuating drugs: Barbiturates, corticosteroids, diuretics
Contraindications
- Not suitable for insulin dependent diabetes mellitus
- Not for diabetic ketoacidosis or diabetic coma
- Not for patients hypersensitive to sulfonylureas
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea
- Vomiting
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary
- Must not be taken by breastfeeding women
Precautions & Warnings
- Initial weeks may increase risk of hypoglycaemia
- Hypoglycaemia can be controlled by immediate intake of carbohydrates
Overdose Effects
- Overdosage can produce hypoglycaemia
- Mild symptoms treated with oral glucose and adjustments in drug dosage
- Severe reactions require immediate hospitalization
