Indications

  • Treatment of hypertension
  • Delaying the progression of renal disease in hypertensive type-2 diabetics with proteinuria

Pharmacology

  • Non-peptide orally active angiotensin II receptor blocker
  • Binds to AT1 receptor in various tissues
  • Reduces vasoconstriction and aldosterone release responsible for hypertension

Dosage & Administration

  • Usual starting and maintenance dose of 50 mg once daily
  • Can be used alone or in combination with other antihypertensive agents
  • Total daily dose ranges from 25 mg to 100 mg
  • Different starting dose for patients over 75 years and with intravascular volume-depletion

Interaction

  • Reduction of active metabolite levels when used with rifampicin and fluconazole
  • Potentiation of antihypertensive effects when used with hydrochlorothiazide
  • Increased serum potassium when used with potassium-sparing diuretics or supplements
  • Potential risk of renal impairment with concomitant use of certain drugs

Contraindications

  • Contraindicated in pregnant women
  • Contraindicated in patients hypersensitive to any component
  • Not to be administered with Aliskiren in diabetic patients

Side Effects

  • Mild and transient side effects
  • Common side effects include dizziness, diarrhea, nasal congestion, cough, and upper respiratory infection
  • Other side effects include fatigue, edema, abdominal pain, chest pain, nausea, headache, and pharyngitis

Pregnancy & Lactation

  • Pregnancy Category D
  • Risk to fetus when administered during second or third trimesters
  • Unknown excretion in human milk
  • Decision should be made whether to discontinue nursing or discontinue the drug

Precautions & Warnings

  • Reduction of fetal renal function and increase in fetal and neonatal morbidity and death during second and third trimesters
  • Symptomatic hypotension in intravascularly volume-depleted patients
  • Significantly increased plasma concentration in cirrhotic patients
  • Changes in renal function including renal failure in renal impaired patients

Use in Special Populations

  • No initial dosage adjustment necessary in patients with mild renal impairment
  • Lower starting dose recommended for patients with moderate to severe renal impairment or on dialysis
  • Lower dose recommended for patients with hepatic impairment
  • Contraindicated in pregnancy and in patients hypersensitive to losartan
