Indications
- Relief of symptoms from allergic rhinitis in adults and children 12 years and older
- Relief of symptoms from chronic idiopathic urticaria in adults and children 12 years and older
Pharmacology
- H1 histamine receptor mediated hypersensitivity and allergic reactions
- Exposure to allergen results in degranulation of mast cells and basophils
- Histamine binds to and activates H1 receptors, resulting in release of pro-inflammatory cytokines
- Fexofenadine acts as an inverse agonist of the H1 receptor
- Fexofenadine has potent and selective affinity for H1 receptors with no evidence of other activity
- Fexofenadine does not cross the blood-brain barrier
Dosage & Administration
- Adults and children 12 years and older: 60 mg twice daily or 120 mg once daily or 180 mg once daily for allergic rhinitis
- Children from 6 to 11 years: 30 mg twice daily or 60 mg once daily for allergic rhinitis
- Children from 2 to 11 years: 30 mg or 5 ml twice daily, dosage adjustment for impaired renal function
- Adults and children 12 years and older: 60 mg twice daily or 120 mg once daily or 180 mg once daily for chronic idiopathic urticaria
- Children from 6 to 11 years: 30 mg twice daily or 60 mg once daily for chronic idiopathic urticaria
- Children from 6 months to less than 2 years: 15 mg or 2.5 ml twice daily, dosage adjustment for impaired renal function
Interaction
- No hepatic biotransformation, no interaction with medicinal products through hepatic mechanisms
- Coadministration with erythromycin or ketoconazole results in 2-3 times increase in fexofenadine level in plasma
- No interaction with omeprazole observed
- Antacid containing aluminium and magnesium hydroxide gels 15 minutes prior to administration causes reduction in bioavailability
Contraindications
- Known hypersensitivity to Fexofenadine Hydrochloride or any of its ingredients
Side Effects
- Nervous system disorders: headache, drowsiness, dizziness
- Gastrointestinal disorders: nausea, diarrhea
- General disorders and administration site conditions: fatigue
- Immune system disorders: hypersensitivity reactions with manifestations such as angioedema, chest tightness, dyspnoea, flushing and systemic anaphylaxis
- Psychiatric disorders: insomnia, nervousness, sleep disorders or nightmares/excessive dreaming
- Cardiac disorders: tachycardia, palpitations
- Skin and subcutaneous tissue disorders: rash, urticaria, pruritus
Pregnancy & Lactation
- No adequate data from use in pregnant women
- Limited animal studies do not indicate direct or indirect harmful effects
- Fexofenadine hydrochloride not recommended for mothers breast-feeding their babies
- No human data on the effect of Fexofenadine hydrochloride on fertility
Precautions & Warnings
- Limited data in older people and renally or hepatically impaired patients
- Caution advised in these special groups
- Patients with history of or ongoing cardiovascular disease warned of adverse reactions like tachycardia and palpitations
- Unlikely to produce effect on ability to drive or use machines
Use in Special Populations
- Pharmacokinetics altered in individuals with renal impairment
- Dose adjustment recommended for patients with decreased renal function
Overdose Effects
- Reported effects: dizziness, drowsiness, fatigue, dry mouth
- No development of clinically significant adverse reactions reported with doses up to 800 mg
- Recommended standard measures for removal of unabsorbed medicinal product
