Indications
- Relief of signs & symptoms of Osteoarthritis
- Relief of signs & symptoms of Rheumatoid arthritis
- Relief of signs & symptoms of Ankylosing spondylitis
- Decrease risk of developing gastric ulcers in patients at risk of NSAID-associated gastric ulcers
Pharmacology
- Consists of immediate release Esomeprazole Magnesium layer & enteric-coated Naproxen core
- Naproxen is a NSAID with analgesic & antipyretic properties
- Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion by inhibition of the H+/k+ -ATPase in gastric parietal cells
Dosage
- Use lowest effective dose for the shortest duration
- Adult dosage: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole twice daily
- Adolescent dosage: 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole for weight greater than 50 kg
Administration
- Do not split, chew, crush or dissolve the tablet
- Take at least 30 minutes before meals
Interaction
- Concomitant use of NSAIDs may reduce the antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
- Concomitant use of tablet and warfarin may result in increased risk of bleeding complication
- Esomeprazole may interfere with the absorption of drugs like Ketoconazole, iron salts and digoxin
Contraindications
- Known hypersensitivity to any component of this tablet
- History of asthma, urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
- Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery
Side Effects
- Decrease occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
- May be associated with a small increased risk of arterial thrombotic events
- Common adverse events are gastrointestinal in nature
Pregnancy & Lactation
- Not recommended during the first and second trimester of pregnancy
- Contraindicated during the third trimester of pregnancy
- Excreted in low quantities in human milk, should not be used during breastfeeding
Precautions & Warnings
- Avoid combination with other NSAIDs
- Minimize undesirable effects by using lowest effective dose for shortest duration
- Risk-factors for developing NSAID-related gastro-intestinal complications should be considered
Use in Special Populations
- Use caution and possible dosage adjustment in elderly patients
- Not recommended for use in patients with moderate to severe or severe renal impairment
- Monitor and consider dose reduction in patients with hepatic insufficiency
Overdose Effects
- Symptoms of overdose include lethargy, dizziness, drowsiness, epigastric pain, abdominal discomfort, heartburn, and more
- Patients should be managed by symptomatic and supportive care following a NSAID overdose
- No specific antidote known for Esomeprazole overdose
