Pharmacology

Paclitaxel promotes the assembly of microtubules, stabilizes microtubules, induces abnormal arrays of microtubules, and inhibits normal dynamic cellular functions.

Dosage & Administration

Premedication required to prevent severe hypersensitivity reactions
Different dosages recommended for various treatments
Dose adjustment based on individual patient tolerance

Interaction

Cimetidine does not affect Taxopac clearance
Cisplatin use can lead to greater myelosuppression
Sequence effects observed with combination use of Taxopac and doxorubicin

Contraindications

Severe hypersensitivity reactions to paclitaxel and macrogolglycerol ricinoleate
Lactation
Baseline neutrophils and platelets below specified levels
Severe hepatic impairment

Side Effects

Common: Low blood counts, hair loss, joint/muscle pain, peripheral neuropathy, nausea, vomiting, diarrhea
Rare: Edema, liver problems, low blood pressure, skin darkening

Pregnancy & Lactation

Pregnancy Category D, contraindicated during lactation

Precautions & Warnings

Administered under supervision of experienced physician
Significant hypersensitivity reactions and extravasation risk
Bone marrow suppression, cardiac conduction abnormalities reported

Use in Special Populations

Dose reduction for severe neutropenia or peripheral neuropathy
Not recommended for pediatric use below 18 years
No dosage alterations recommended for mild to moderate hepatic impairments

Overdose Effects

No known antidote for overdose, monitor closely and treat anticipated toxicities

Reconstitution

Taxopac Injection must be diluted prior to infusion, solutions are stable for up to 27 hours at ambient temperature

Storage Conditions

Store below 30°C, protect from light, use freshly prepared solution
