Indications
- Reducing elevated total cholesterol
- Lowering low-density lipoprotein cholesterol (LDL-C)
- Increasing high-density lipoprotein cholesterol (HDL-C)
- Reduction of elevated triglycerides (TG)

- Treatment for primary hyperlipidemia

Pharmacology
- Lowers plasma cholesterol
- Inhibits HMG-CoA reductase
- Reduces LDL production
- Increases hepatic LDL receptors
- Inhibits absorption of cholesterol by small intestine
- Reduces blood cholesterol levels
- Does not inhibit cholesterol synthesis in the liver
Dosage & Administration
- Dosage range: 10/10 mg to 80/20 mg daily
- Recommended starting dose: 10/10 mg or 20/10 mg daily
- Administered as a single dose with or without food
- Lipid levels analysis within 2 or more weeks post initiation
- Dosage adjustment based on lipid levels
Interactions
- Increased risk of myopathy with concurrent fibric acid derivatives
- Enhanced risk of skeletal muscle effects with niacin
- Monitoring required when coadministered with digoxin
- Caution required when coadministered with colchicine or warfarin
Contraindications
- Active liver disease
- Hypersensitivity to any component of the preparation
Side Effects
- Rhabdomyolysis
- Myopathy

Pregnancy & Lactation
- Contraindicated in pregnancy
- Lipid-lowering drugs offer no benefit during pregnancy
- Impact on long-term outcomes of primary hypercholesterolemia therapy is minimal if discontinued during pregnancy
- Unknown excretion in human milk
Precautions & Warnings
- Temporary withholding or discontinuation in case of myopathy
- Liver enzyme tests prior to initiating therapy
- Caution in case of liver injury or unexplained transaminase elevations
- Exercise caution when administered with drugs decreasing endogenous steroid hormone levels
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No dosage adjustment necessary in geriatric patients
