Indications
- Treatment of essential hypertension in adults
- Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease
- Reduction of cardiovascular morbidity in adults with type 2 diabetes mellitus
- Reduction of cardiovascular morbidity in adults with documented target organ damage
Pharmacology
- Non-peptide angiotensin-II receptor antagonist
- Blocks vasoconstrictor and aldosterone secreting effects of angiotensin-II
- Selective blocking of angiotensin-II binding to the AT1 receptor
- Greater affinity for the AT1 receptor than for the AT2 receptor
- Does not inhibit ACE (kininase-II)
Dosage & Administration
- Usual starting dose of 40 mg once a day
- Blood pressure response is dose-related over the range of 20 to 80 mg
- Antihypertensive effect is apparent within 2 weeks
- Maximal reduction generally attained after 4 weeks
- May be administered with other antihypertensive agents
Interaction
- Increased risk of renal impairment and loss of antihypertensive effect with NSAIDs
- Co-administration with aliskiren in patients with diabetes should be avoided
Contraindications
- Known hypersensitivity to the product or any of its components
Side Effects
- Common side effects in hypertensive patients: sinus pain, congestion (sinusitis), back pain, diarrhea
- Common side effects in patients for cardiovascular risk reduction: intermittent claudication, skin ulcer
Pregnancy & Lactation
- Pregnancy Category C (first trimester) and D (second and third trimester)
- Decision on nursing or drug discontinuation to be made considering the importance of the drug to the mother
Precautions & Warnings
- Avoid fetal or neonatal exposure
- Hypotension
- Monitor carefully in patients with impaired hepatic or renal function
- Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker
Overdose Effects
- Manifestations: hypotension, dizziness, tachycardia
- Possible increase in serum creatinine and acute renal failure
