Dosage
- Hypertension:
- 5 mg once daily
- Max 10 mg once daily
- 2.5 mg for elderly patients with hepatic insufficiency
- Angina:
- 5-10 mg
- Lower dose for elderly and patients with hepatic insufficiency
- Most patients require 10 mg
- Administration: May be taken without regard to meals
Drug Interactions
- Potentially hazardous interactions: Avoid combination with p-blockers in patients with markedly impaired left ventricular function

- Hypersensitivity to dihydropyridine derivatives
- Pregnant women

Use in Special Populations
- Children with hypertension:
- 6-17 years of age
- 2.5 mg once daily as starting dose, up-titrated to 5 mg once daily if needed
- Children under 6 years old: Effect on blood pressure not known
- Elderly: Equally well tolerated at similar doses
- Renal impairment: Normal dosage recommended
- Hepatic impairment:
- Dosage recommendations not established in mild to moderate cases
- Lower dose initiation in severe cases
Overdose Effects
- Symptoms: Excessive peripheral vasodilatation, reflex tachycardia, systemic hypotension, shock
- Management:
- Active cardiovascular support
- Frequent monitoring of cardiac and respiratory function
- Elevation of extremities
- Attention to circulating fluid volume and urine output
- Use of vasoconstrictor
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile in some cases
- Charcoal administration
- Dialysis unlikely to be beneficial due to high protein binding
