Indications

Pharmacology

  • Cyclic polypeptide consisting of 11 amino acids
  • Potent immunosuppressive agent
  • Prolongs survival of allogeneic transplants
  • Acts specifically and reversibly on lymphocytes
  • Does not depress haemopoiesis
  • No effect on the function of phagocytic cells

Dosage & Administration

    • Solid organ transplantation: Initial dose: 10 to 15 mg/kg
    • Bone marrow transplantation: Initial dose: 12.5 to 15 mg/kg
    • Endogenous uveitis: Initial dose: 5 mg/kg per day
    • Nephrotic syndrome: Initial dose: 5 mg/kg for adults, 6 mg/kg for children
    • Rheumatoid arthritis: Recommended dose: 3 mg/kg per day for first 6 weeks
    • Psoriasis & Atopic dermatitis: Initial dose: 2.5 mg/kg per day

Interaction

  • Drugs that decrease Neoral levels: Barbiturates, carbamazepine, oxcarbazepine, phenytoin, etc.
  • Drugs that increase Neoral levels: Macrolide antibiotics, ketoconazole, fluconazole, etc.
  • Other relevant drug interactions: Neoral may reduce the clearance of digoxin, colchicine, prednisolone, etc.

Contraindications

  • Hypersensitivity to ciclosporin or to any of the excipients of Sandimmun Neoral
  • Kidney failure (except in patients with nephrotic syndrome)
  • Uncontrolled hypertension
  • Uncontrolled infection
  • History of known or diagnosed malignancy

  • Skin reactions

Pregnancy & Lactation

Not recommended during pregnancy or breastfeeding

Precautions & Warnings

  • Increases risk of developing lymphomas and other malignancies
  • Avoid excess ultraviolet light exposure
  • May develop bacterial, fungal, parasitic and viral infections
  • Reversible increase in serum creatinine and urea
  • Regular monitoring of blood pressure required
  • Enhances risk of hyperkalaemia
  • Vaccination may be less effective

Overdose Effects

Renal dysfunction, which resolves following drug withdrawal
