Indications
- Treatment of acute and chronic psychoses
- Treatment of schizophrenia
- Treatment of bipolar disorder
- Maintenance treatment of bipolar I disorder
- Prevention of recurrence of manic, depressive, or mixed episodes
Pharmacology
- Atypical psychotropic agent
- Antagonist at neurotransmitter receptors
- No appreciable affinity at cholinergic muscarinic and benzodiazepine receptors
- Mechanism of action is unknown
- Efficacy in schizophrenia mediated through dopamine D2 and serotonin 5HT2 antagonism
Dosage & Administration
- Twice daily administration
- Administered with or without food
- Initial titration dosage for acute and chronic psychoses
- Initial titration dosage for manic episodes associated with bipolar disorder
- Initial titration dosage for depressive episodes associated with bipolar disorder
- Usual effective dose range of 300-450 mg/day for schizophrenia
- Usual effective dose range of 400-800 mg/day for manic episodes
- Usual dose of 300 mg/day for depressive episodes
- Combination therapy maintenance dose range of 400-800 mg/day
Interaction
- Caution with medicines causing electrolyte imbalance
- Caution with medicines increasing QT interval
- Co-administration causing increases in clearance of the drug
- Caution with microsomal enzyme inducers
Contraindications
- Contra-indicated in patients hypersensitive to the drug
Side Effects
- Somnolence
- Dizziness
- Dry mouth
- Withdrawal symptoms
- Elevations in serum triglyceride levels
- Elevations in total cholesterol
- Decreases in HDL cholesterol
- Weight gain
- Decreased haemoglobin
- Extrapyramidal symptoms
Pregnancy & Lactation
- Safety and efficacy not established in human pregnancy
- Use during pregnancy only if benefits justify the potential risks
- Avoid breastfeeding while taking the drug
Precautions & Warnings
- Increased risk of suicidal thoughts and self-harm
- Caution in patients with cardiovascular disease or predisposing conditions
- Caution in patients with history of seizures
- Consideration of dose reduction or discontinuation if tardive dyskinesia appears
- Discontinue drug if neuroleptic malignant syndrome appears
- Caution in patients with family history of QT prolongation
- Risk of severe neutropenia
- Observation of blood glucose levels and occurrence of diabetes
Use in Special Populations
- Caution in elderly patients
- Lower initial dosing and slower titration in elderly
- Not indicated for use in children and adolescents below 18 years of age
Overdose Effects
- Survival reported in acute overdoses of up to 30 grams
- No specific antidote
- Consideration of multiple drug involvement
- Intensive care procedures recommended
- Treatment of refractory hypotension with appropriate measures
- Close medical supervision and monitoring
