Indications
- Treatment of type 2 diabetes mellitus in adults as adjunct to diet and exercise
- Adjunct therapy in patients insufficiently controlled on maximally tolerated dose of Metformin alone
- Adjunct therapy in combination with other medicinal products for diabetes treatment
- Adjunct therapy in patients already being treated with the combination of Empagliflozin and Metformin as separate tablets
Pharmacology
- Empagliflozin inhibits Sodium-Glucose Co-Transporter 2 (SGLT2)
- Metformin Hydrochloride is a biguanide type oral antihyperglycemic drug
- Lowering of basal and postprandial plasma glucose
- Reduction of renal reabsorption of filtered glucose
- Increases urinary glucose excretion
Dosage & Administration
- Individualized dosage based on effectiveness and tolerability
- Twice daily administration with meals
- Gradual dose escalation to reduce gastrointestinal side effects
- Maximum recommended daily dose of Metformin Hydrochloride is 2000 mg and Empagliflozin is 25 mg
- Recommended individualized starting dose based on patient's drug regimen
- Dose adjustment for renal impaired patients
- Extended-release formulations available
- Dosing adjustment for extended-release formulations
- Pediatric patients under 18 years of age are not recommended
Interaction
- Co-administration with diuretics increases urine volume and frequency of voids
- Co-administration with insulin or insulin secretagogues increases the risk for hypoglycemia
- Monitoring glycemic control with urine glucose tests not recommended
- Drugs that reduce Metformin clearance may increase the accumulation of Metformin
- Carbonic Anhydrase Inhibitors may increase risk of lactic acidosis
- Drugs affecting glycemic control need careful monitoring
- Alcohol intake can potentiate the effect of Metformin on lactate metabolism
Contraindications
- Hypersensitivity to Empagliflozin and Metformin
- Acute metabolic acidosis (lactic acidosis, diabetic ketoacidosis)
- Severe renal failure
- Conditions altering renal function such as dehydration, severe infection, shock
- Disease causing tissue hypoxia such as heart failure, respiratory failure, recent myocardial infarction, shock
- Hepatic impairment, acute alcohol intoxication, alcoholism
Side Effects
- Most common adverse reactions associated with Empagliflozin include urinary tract infection and female genital mycotic infections
- Most common adverse reactions associated with Metformin include diarrhea, nausea/vomiting, flatulence, abdominal discomfort, indigestion, asthenia, headache
- Hypoglycemia, Gastrointestinal symptoms, Vaginal moniliasis, vulvovaginitis, balanitis, urinary tract infection, thirst, taste disturbance, pruritus, rash, Increased urination, serum lipids increased, Volume depletion, urticaria, dysuria, blood creatinine increased/Glomerular filtration rate decreased, Haematocrit increased, Diabetic ketoacidosis
Pregnancy & Lactation
- Potential risk to fetus especially during the second and third trimesters
- Not recommended during breastfeeding
Precautions & Warnings
- Risk of Lactic Acidosis
- Hypotension in certain patient groups
- Risk of Ketoacidosis
- Acute kidney injury and impairment in renal function
- Increased risk for urosepsis, pyelonephritis, Fournier's gangrene, and genital mycotic infections
- Risk of Hypoglycemia
- Potential for Vitamin B12 Deficiency
- Monitoring and treatment for increased LDL-C
- No conclusive evidence of macrovascular risk reduction
Overdose Effects
- No toxicity observed at high single doses of Empagliflozin
- Lactic acidosis reported with high Metformin doses
- Medical emergency in case of lactic acidosis
- Treatment should be based on patient's clinical status
- Haemodialysis for removal of lactate and Metformin
