Indications
- Treatment of hypertension
- Lowering blood pressure
- Initial therapy for multiple antihypertensive agents
Pharmacology
- Calcium channel blocker
- Vasodilator
- Aldosterone secretion blocker
- Competitive inhibitor of the AT1 receptor
- Inhibition of ACE
- Potent vasoconstrictor
Dosage & Administration
- Substitute individually titrated components
- Initial therapy with 5/20 mg once daily
- Titration to maximum of 10/40 mg once daily
- Starting dose of Amlodipine is 2.5 mg for patients 75 years and older
Interaction
- Attenuation of antihypertensive effect by NSAIDs including selective COX-2 inhibitors
- Close monitoring of blood pressure, renal function, and electrolytes for combination therapy
Contraindications
- Co-administration contraindicated with Aliskiren in patients with diabetes

Pregnancy & Lactation
- Pregnancy Category D
- Risk of fetal death in 2nd and 3rd trimester
- Unestablished excretion in human milk
- Consideration for discontinuation of nursing or drug
Precautions & Warnings
- Caution for hypotension in volume- or salt-depleted patients
- Vasodilation in severe aortic stenosis patients
- Angina or acute MI risk in severe obstructive coronary artery disease patients
Use in Special Populations
- Unestablished safety and effectiveness in pediatric patients
- No overall differences in safety or effectiveness in subjects 65 years of age or older
- No studies in patients with renal impairment
- Not recommended for hepatically impaired patients
