Indications
- Treatment of adults with type 2 diabetes mellitus
- Adjunct to diet and exercise
- Insufficiently controlled on Metformin alone
- Combination with other medicinal products for diabetes
- Usable for patients on Empagliflozin and Metformin separately
Pharmacology
- Inhibitor of Sodium-Glucose Co-Transporter 2 (SGLT2)
- Reduces renal reabsorption of filtered glucose
- Lowers renal threshold for glucose
- Increases urinary glucose excretion
- Biguanide type oral antihyperglycemic drug
- Lowers basal and postprandial plasma glucose
- Increases peripheral glucose uptake and utilization
Dosage & Administration
- Individualized dosage based on effectiveness and tolerability
- Take twice daily with meals
- Maximum recommended daily doses: 2000 mg Metformin, 25 mg Empagliflozin
- Extended-release formulations available
- Dosing adjustment for renal impaired patients
- Not recommended for pediatric patients under 18 years of age
Interaction
- Increased urine volume and frequency with diuretics
- Increased risk of hypoglycemia with insulin or insulin secretagogues
- Monitoring glycemic control with urine glucose test not recommended
- Drugs that reduce Metformin clearance may increase accumulation
- Carbonic Anhydrase Inhibitors may increase risk of lactic acidosis
- Other drugs affecting glycemic control
Contraindications
- Hypersensitivity to Empagliflozin and Metformin
- Acute metabolic acidosis
- Severe renal failure (GFR < 30 ml/min)
- Acute conditions altering renal function
- Diseases causing tissue hypoxia
- Hepatic impairment, acute alcohol intoxication, alcoholism

- Increased blood creatinine/Glomerular filtration rate
- Rare: Diabetic ketoacidosis
Pregnancy & Lactation
- Potential risk to fetus during second and third trimesters
- Not recommended during breastfeeding

- Acute kidney injury & impairment
- Urosepsis, Pyelonephritis, Fournier’s gangrene & Genital mycotic infections
- Hypoglycemia risk
- Vitamin B12 Deficiency
- Increased LDL-C
- Lack of evidence for macrovascular risk reduction
Overdose Effects
- No toxicity observed at high single or multiple doses
- Lactic acidosis risk with high Metformin doses
- Hypoglycemia not seen with Metformin overdoses
- Medical emergency requiring hospital treatment
- Haemodialysis for lactate and Metformin removal
