Indications
- Lowering blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
- Adjunct to diet and exercise
- Used in combination with metformin and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Initial dose 1 mg once daily
- Dose titration based on regular blood sugar monitoring
- Usual dose range 1 to 4 mg daily
- Single daily dose usually taken before a substantial breakfast
- Dose adjustment required with changes in weight or lifestyle
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiates hypoglycaemic action:
- ACE inhibitors
- Chloramphenicol
- Fluconazole
- Miconazole
- Attenuates hypoglycaemic action:
- Barbiturates
- Calcium channel blockers
- Corticosteroids
- Thyroid hormones
- May lead to either potentiation or weakening of the blood-glucose-lowering effect:
- H2 receptor antagonists
- Beta-blockers
- Clonidine
- Reserpine
- May mask the warning symptoms of a hypoglycaemic attack:
- Beta-receptor blockers
- Clonidine
- Guanethidine
- Reserpine
- Effects of alcohol intake:
- May potentiate or attenuate the activity of Glymax in an unpredictable fashion
Contraindications
- Insulin dependent (type I) diabetes mellitus
- Diabetic ketoacidosis
- Diabetic coma
- Hypersensitivity to Glimepiride
- Severe hepatic dysfunction
- Severe impairment of renal function
- Dialysis patients

- Hypoglycaemia
- Temporary visual impairment
- Nausea
- Vomiting
- Diarrhoea
- Abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary
- Not to be taken by breastfeeding women
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Necessitates careful monitoring
- Prompt control with immediate intake of carbohydrates
Overdose Effects
- Can produce hypoglycaemia
- Mild symptoms treated with oral glucose and adjustments in drug dosage or meal patterns
- Severe reactions with coma or seizure require immediate hospitalization
