Indications
- Lower the blood glucose in patients with noninsulin dependent (Type II) diabetes mellitus (NIDDM)
- Adjunct to diet and exercise
- Can be used concomitantly with metformin
- Can be used in combination with insulin
- Pharmacological effect lasts for 24 hours
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Initial dose is 1 mg once daily
- Daily dose can be increased gradually based on regular blood sugar monitoring
- Usual dose range in well controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses decided by physician
- Must not be skipped after taking the drug
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiating drugs include ACE inhibitors, aminosalicylic acid, anabolic steroids, and more
- Attenuating drugs include acetazolamide, barbiturates, calcium channel blockers, and more
- Concomitant treatment with certain drugs may mask the warning symptoms of hypoglycemia
- Alcohol intake may affect the activity of the medicine
Contraindications
- Not suitable for insulin dependent (type I) diabetes mellitus
- Not to be used in diabetic ketoacidosis or diabetic coma
- Must not be used in patients hypersensitive to the drug or other sulfonylureas

- Fall in blood pressure
Pregnancy & Lactation
- Must not be taken during pregnancy
- Ingestion with breast milk feeding may harm the child
- Patients planning a pregnancy should change over to insulin
Precautions & Warnings
- Initial weeks of treatment require careful monitoring for hypoglycemia
- Dosage adjustment may be necessary to control hypoglycemia
- Hypoglycemia can be promptly controlled by immediate intake of carbohydrates
Overdose Effects
- Overdosage can produce hypoglycemia
- Mild hypoglycemic symptoms should be treated with oral glucose and dosage adjustments
- Severe hypoglycemic reactions with coma or seizure require immediate hospitalization
