Indications
- Treatment for Anaphylaxis
- Management of asthma
- Rheumatoid arthritis therapy
- Treatment for lupus erythematosus
- Management of ulcerative colitis
Pharmacology
Inhibits Phospholipase A2 enzyme for anti-inflammatory action, decreases release of immune system chemicals for immunosuppressive action
Dosage & Administration
- Adults:
- Maintenance doses between 3-18 mg/day for most conditions
- Initial dose of up to 120 mg/day for acute disorders
- Children:
- Usual maintenance dose between 0.25-1.0 mg/kg/day
- Dose for juvenile chronic arthritis lies in the range 0.25-1.0 mg/kg/day
Interaction
Metabolized in the liver, maintenance dose adjustment required when co-administered with liver enzyme inducers or inhibitors
Contraindications
Hypersensitivity to any of the ingredients, caution advised for patients receiving live virus immunization

Pregnancy & Lactation

- Nursing Mother: Excreted in breast milk, doses of up to 50 mg daily unlikely to cause systemic effects in the infant

- Conditions requiring caution:
- Cardiac disease or congestive heart failure
- Hypertension
- Thromboembolic disorders
- Gastritis or oesophagitis
- Ulcerative colitis
- Diabetes mellitus
- Osteoporosis
- Myasthenia gravis
- Renal insufficiency
- Emotional instability or psychotic tendency
- Epilepsy
- Liver failure
- Hypothyroidism and cirrhosis
- Ocular herpes simplex
Use in Special Populations
- Hepatic Impairment: Blood levels may be increased, careful monitoring and adjustment of dose required
- Renal Impairment: No special precautions necessary
- Elderly: Adverse effects associated with systemic corticosteroids may have more serious consequences in old age
