Indications
- Post-menopausal osteoporosis
- Renal osteodystrophy
- Postsurgical hypoparathyroidism
- Idiopathic hypoparathyroidism
- Pseudohypoparathyroidism
- Secondary hyperparathyroidism in patients with chronic renal failure
- Vitamin D dependent rickets
Pharmacology
Calcitriol is one of the most important active metabolites of vitamin D3. It promotes intestinal absorption of calcium and regulates bone mineralization.
Dosage & Administration
- Recommended dose for post-menopausal osteoporosis: 0.25 mcg twice daily
- Initial dose for renal osteodystrophy: 0.25 mcg daily or 0.25 mcg every other day for patients with normal calcium levels
- Recommended initial dosage for hypoparathyroidism and rickets: 0.25 mcg/day, increased if necessary
- Intravenous initial dose: 1-2 mcg administered three times weekly
- Dosage adjustments for pediatric and elderly patients
Interaction
Concomitant treatment with thiazide diuretics increases the risk of hypercalcemia. Careful dosage determination in patients undergoing treatment with digitalis. Magnesium containing drugs may cause hypermagnesemia.
Contraindications
Hypersensitivity to any of its ingredients. Contraindicated in diseases associated with hypercalcemia.
Side Effects
- Early side effects: weakness, headache, nausea, bone pain
- Late side effects: polyuria, anorexia, conjunctivitis, hypercholesterolemia
- Occasional mild pain on injection
Pregnancy & Lactation
No evidence of teratogenicity. Use during pregnancy if benefits outweigh potential risk. Mothers may breastfeed while taking Calcitriol with monitored serum calcium levels.
Precautions & Warnings
Excessive dosage induces hypercalcemia and hypercalciuria. Caution in patients on digitalis. Discontinuation may result in rebound effect.
Use in Special Populations
- Dosage for pediatric patients
- No dosage adjustments required for the elderly.
Overdose Effects
Can cause hypercalcemia, hypercalciuria, and hyperphosphatemia. Treatment includes discontinuation of therapy and low calcium diet.
