Indications
- Management of hypertension
- Management of angina pectoris
- Treatment of cardiac arrhythmias
- Adjunct treatment of hyperthyroidism
- Reduction in infarct size in acute myocardial infarction
Pharmacology
- Selective beta1-blocker
- Inhibits the agonistic effect of catecholamines on the heart
- Reduces heart rate, cardiac output, contractility, and blood pressure
- Interferes less with insulin release and carbohydrate metabolism
- Interferes much less with cardiovascular response to hypoglycemia
Dosage & Administration
- Total daily dosage ranges from 100-400 mg for hypertension
- Initial dose of 100 mg per day for hypertension
- Increased dosage for uncontrolled hypertension
- 50-100 mg for angina pectoris
- Up to 300 mg per day for cardiac arrhythmias
- 50 mg four times a day for hyperthyroidism
- Oral therapy for myocardial infarction
- Extended-release tablet dosage varies for different conditions
- IV Injection dosage for arrhythmias and myocardial infarction
Interaction
- Additive effect with catecholamine‐depleting drugs
- Increased concentration with drugs inhibiting CYP2D6
- Risk of bradycardia when used with digitalis glycosides
- Potential exacerbation of rebound hypertension with clonidine withdrawal
Contraindications
- AV block
- Uncontrolled heart failure
- Severe bradycardia
- Sick-sinus syndrome
- Cardiogenic shock
- Severe peripheral arterial disease
- Hypersensitivity to Metoprolol or other B-blockers
- Complicated myocardial infarction with specific conditions
