Indications
- Facilitate endotracheal intubation
- Aid in mechanical ventilation
- Assist in surgical and obstetric procedures
Mode of Action
- Acts as a depolarizing neuromuscular blocker
- Imitates the action of acetylcholine at the neuromuscular junction
- Binds to muscle type nicotinic receptors
- Results in opening of nicotinic sodium channel
- Causes disorganized depolarization of motor-end plate
- Results in release of calcium from sarcoplasmic reticulum
- Leads to fasciculation and muscle flaccidity
Dosage and Administration
- Administered by bolus Intravenous or Intramuscular injection
- Dose dependent on body weight and degree of muscular relaxation required
- Different doses for adults and children
- May be given by intravenous infusion for prolonged surgical procedures
- Total dose given should not exceed 500 mg per hour
Interaction
- Certain drugs or chemicals known to reduce normal plasma cholinesterase activity may prolong neuromuscular blocking effects
- Includes specific anticholinesterase agents, cytotoxic compounds, psychiatric drugs, anesthetic agents, and drugs
Contraindications
- Not to be administered to conscious patients
- Not to be given to patients hypersensitive to the drug or with atypical plasma cholinesterase activity
- Contraindicated in patients recovering from major trauma, severe burns, neurological deficits, pre-existing hyperkalemia, and open eye injuries
- Injection is contraindicated in newborn infants
Side Effects
- Cardiovascular effects like bradycardia, tachycardia, hypertension, hypotension, arrhythmias
- Respiratory effects like bronchospasm, prolonged respiratory depression, apnea
- Musculoskeletal effects like muscle fasciculation, post-operative muscle pains, myoglobinemia
- Other effects like hyperthermia, increased intra-ocular pressure, increased intra-gastric pressure, rash, excessive salivation
Pregnancy and Lactation
- Should not be administered to pregnant women unless potential benefit outweighs possible hazards
Precautions and Warnings
- Should be administered by or under close supervision of an anesthetist familiar with its action and hazards
- Elderly may be more susceptible to cardiac arrhythmias, especially if digitalis-like drugs are also being taken
