Indications

Pharmacology

- Inhibits calcium ion influx into vascular smooth muscle and cardiac muscle

- Greater effect on vascular smooth muscle cells than on cardiac muscle cells
- Tmax of 6-12h
- Bioavailability of 64-90%
- Volume of distribution of 21 L/kg
- Metabolized to inactive metabolites
- Metabolism primarily mediated by CYP3A4
- 60% elimination in urine
Dosage & Administration
- Oral route of administration
- Usual initial antihypertensive oral dose of 2.5 mg once daily
- Maximum dose of 5 mg once daily
- Adjusted dosage for elderly or hepatically impaired patients
- Titration steps with 7-14 days waiting period
- Effective antihypertensive oral dose for pediatric patients ages 6-17 years
Interaction
- Effect of CYP3A inhibitors on amlodipine

- Co-administration of simvastatin with amlodipine increases systemic exposure

Contraindications
- Contraindicated in patients with known sensitivity to amlodipine
Side Effects
- Common side effects include difficulty in breathing, dizziness, reflex tachycardia, hypotension, abdominal pain, and edema
- Rare side effects include diarrhea, back pain, headache, fatigue, and nausea
Pregnancy & Lactation
- Insufficient data for risk assessment in pregnant women
- No observed adverse effects on breastfed infants
Precautions & Warnings
- Symptomatic hypotension is possible
- Worsening angina and acute myocardial infarction risk
- Titration caution for patients with hepatic impairment
