Barinib 2 mg (Tablet)

Indications

• Treatment of moderate to severely active Rheumatoid Arthritis in adult patients
• Inadequate response to one or more tumor necrosis factor (TNF) antagonist therapies

Pharmacology

• Selective and reversible inhibitor of Janus kinase JAK1 and JAK2
• Modulates intracellular signaling pathways from cell surface receptors for cytokines and growth factors
• Partially inhibits JAK1 and JAK2 enzymatic activity

Dosage & Administration

• Recommended adult dose of 2 mg once daily
• Can be used as monotherapy or in combination with Methotrexate or other Disease-modifying antirheumatic drugs (DMARDS)
• Oral administration with or without food
• Not established for pediatric use
• Careful dose selection in geriatric patients
• No dose adjustment for patients with mild or moderate hepatic impairment
• Not recommended for patients with estimated GFR of less than 60 mL/min/I .73 m2

Interaction

• Increased exposure when co-administered with strong OAT3 inhibitors
• Not studied in combination with other JAK inhibitors or with biologic DMARDS

Contraindications

• Avoid initiation or interrupt in patients with Anemia (hemoglobin less than 8 g/dL)
• Lymphopenia (Absolute Lymphocyte Count less than 500 cells/mm³)
• Neutropenia (Absolute Neutrophil Count less than 1000 cells/mm³)

Side Effects

• Commonly reported adverse drug reactions (ADRs) were increased LDL cholesterol, upper respiratory tract infections, and nausea

Pregnancy & Lactation

• Contraindicated during pregnancy
• No information available on the presence in human milk

Precautions & Warnings

• Avoid use in patients with serious infections or active TB
• Consider risks and benefits in patients with known malignancy or at increased risk of thrombosis or gastrointestinal perforation
• To be dispensed only by the prescription of a registered physician

Overdose Effects

• Patients should be monitored for signs and symptoms of adverse reactions
• Appropriate treatment for patients who develop adverse reactions
