Indications

  • Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus

Pharmacology

  • Linagliptin inhibits DPP-4 enzyme
  • Linagliptin increases active incretin hormones
  • Metformin lowers both basal and postprandial plasma glucose
  • Metformin decreases hepatic glucose production

Dosage & Administration

  • Individualized dosage based on effectiveness and tolerability
  • Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
  • Recommended starting dose for different patient groups

Interaction

  • Potential interaction with cationic drugs
  • Contraindicated with carbonic anhydrase inhibitors
  • Rifampin decreases Linagliptin exposure

Contraindications

  • Contraindicated in patients with renal impairment
  • Contraindicated in acute or chronic metabolic acidosis
  • Contraindicated in hypersensitivity to Linagliptin or Metformin Hydrochloride

Side Effects

  • Most common side effects include nasopharyngitis and diarrhea
  • Hypoglycemia is more common in patients treated with Linamet ER and sulfonylureas

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women
  • Caution advised when used during pregnancy and lactation

Precautions & Warnings

  • Discontinue in case of lactic acidosis or pancreatitis
  • Monitoring of hematologic parameters advised
  • Guidance on alcohol consumption, other medicines, counseling, travel, and emergency preparedness
  • Potential for hypoglycemia

Overdose Effects

  • Usual supportive measures in case of overdose
  • No dose-related clinical adverse drug reactions up to 600 mg of Linagliptin
