Description
- Reduces cholesterol saturation of bile
- Promotes the dissolution of gallstones
- Reduces cholesterol secretion into bile
- Increases bile acid secretion rate
Pharmacology
- Decreases secretion of cholesterol in bile
- Reduces cholesterol absorption
- Suppresses liver cholesterol synthesis
- Promotes formation of liquid cholesterol crystal complexes
- Enhances removal of cholesterol from gallbladder
- Protects cholangiocytes against cytotoxicity
- Stimulates hepatobilliary secretion
- Protects hepatocytes against bile acid-induced apoptosis
- Completely absorbed in the upper intestine
- Peak serum concentration varies from 30 to 150 minutes
- Absorption rate ranges from 60-80%
- Enters the portal vein and undergoes extraction from portal blood by liver
- Conjugated with amino acid (glycine or taurine)
- Excreted into hepatic bile ducts
- Appears in the circulation in small quantities
- Biologic half-life ranges from 3.5-5.8 days
Dosage & Administration
- Dissolution of Gall stones: 8-12 mg/kg/day
- Primary Billiary Cirrhosis: 10-15 mg/kg/day
- Acute Viral Hepatitis: 600 mg/day
- Alcoholic Fatty Liver: 300 mg/day
- Primary Sclerosing Cholangitis: 25-30 mg/kg/day
- Dissolution of Gallstones and Non-Alcoholic Steato Hepatitis: 13-15 mg/kg/day
Interaction
- Should not be used with oestrogenic hormones
- Avoid concomitant administration with bile-acid binding drugs
Contraindications
- Non-functioning gall-bladder
- Calcified and pigmented gallstones
- Inflammatory bowel disease
Side Effects
- Nausea
- Vomiting
- Diarrhoea
- Gallstone opacilication
- Pruritus
Pregnancy & Lactation
- Pregnancy category B
- No evidence of harm reported in pregnancy
- Effectively used for the treatment of cholestasis of pregnancy
- Problems not documented regarding breastfeeding
