Indications
- Treatment for severe forms of acne
- Indicated for nodular or conglobate acne
- Resistant to standard therapy with systemic antibacterials and topical therapy
Pharmacology
- Alters cell cycle and differentiation
- Reduces sebum production
- Prevents blockage of pores
- Reduces growth of acne causing bacteria
- Little to no affinity for retinol binding proteins
- Induces apoptosis in sebocytes
- Reduces the formation of comedones
- Does not directly kill bacteria
Dosage & Administration
- Taken once or twice daily with food
- Initial dose of 0.5 mg/kg daily
- Dosage adjustment during therapy
- Long-term remission related to total dose administered
- Treatment duration of 16-24 weeks
- Lower starting dose for severe renal insufficiency
- Not indicated for prepubertal acne
- Continued lower dose for intolerance
Interaction
- Additive adverse effects with vit A or its derivatives
- Decreased efficacy of microdosed progesterone
- Increased risk of local irritation with topical keratolytic or exfoliative anti-acne agents
- Reduced efficacy with oxidising agents
Contraindications
- Contraindicated in pregnant or breastfeeding women
- Contraindicated in patients with hypersensitivity to Isotretinoin or excipients
- Contraindicated in patients with hepatic insufficiency
- Contraindicated in patients with excessively elevated blood lipid values
- Contraindicated in patients receiving concurrent treatment with tetracyclines
