Indications
- Adjunct in the treatment of parkinsonism
- Adjuvant therapy for treating parkinsonism with levodopa
- Control of extrapyramidal disorders caused by central nervous system drugs
Description
- Exerts direct inhibitory effect upon the parasympathetic nervous system
- Relaxing effect on smooth musculature
- Inhibitory effect upon the parasympathetic nervous system
Pharmacology
- Non-selective muscarinic acetylcholine receptor antagonist
- Higher affinity for M1 subtype
- Higher affinity for central muscarinic receptors in the cerebral cortex
- Lower affinity for peripherally located muscarinic receptors
Dosage & Administration
- Dosage should be individualized
- Initial dose should be low and increased gradually
- Total daily dose depends on optimal level
- Different dosages for idiopathic and drug-induced parkinsonism
- Concomitant use with levodopa or other Parasympathetic Inhibitors
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with concurrent use of alcohol or other CNS depressants
- Intensified anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Increased risk of tardive dyskinesia during concomitant administration of anticholinergics and neuroleptics

- Hypersensitivity to Trihexyphenidyl
- Narrow angle glaucoma

Pregnancy & Lactation
- Use during pregnancy only when need clearly established and benefit outweighs risk
- Caution in nursing women due to potential risk to the infant
- May cause suppression of lactation
Precautions & Warnings
- Gonioscope evaluation prior to therapy
- Close monitoring of intraocular pressures
- Precipitation of angle closure with increase in intraocular pressure
- Administration caution in hot weather
- Consideration of narrow angle glaucoma if blurring of vision occurs
- Anhidrosis may occur more readily in certain conditions
- Potential for severe anhidrosis and fatal hyperthermia
- Risk of Neuroleptic Malignant Syndrome
Overdose Effects
- Typical central symptoms of atropine intoxication
- Peripheral signs of parasympathetic blockade
- Neuropsychiatric signs
- Progression to stupor, coma, paralysis, cardiac and respiratory arrest
- Treatment involves symptomatic and supportive therapy
- Gastric lavage or other methods to limit absorption
- Respiratory support, artificial respiration or vasopressor agents may be necessary
- It is not known if Tripar is dialyzable
