Description
- Adorbis is indicated for hypertension, angina, and moderate to severe heart failure. It is a beta-blocker medication used to lower blood pressure, reduce the workload on the heart, and prevent chest pain. The pharmacology of Bisoprolol Hemifumarate involves selective ß1 adrenergic receptor blocking and reduction of heart rate and cardiac output. It is available in tablet form with a recommended starting dose of 5 mg once daily for adults, and caution is advised for patients with renal or hepatic impairment. Adorbis is contraindicated in patients with cardiogenic shock, overt heart failure, second or third degree A-V block, right ventricular failure secondary to pulmonary hypertension, and sinus bradycardia. Patients using Adorbis should avoid combining it with other β-blocking agents and be monitored closely when receiving catecholamine-depleting drugs or centrally active antihypertensive agents. Adorbis may produce side effects such as fatigue, dizziness, headache, disturbances of the gut, cold or numb extremities, muscle weakness or cramps, and breathing difficulties. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus, and nursing mothers should consider stopping nursing if the use of Adorbis is essential. Caution is advised in adjusting the dose of Adorbis in patients with renal or hepatic impairment, and patients with a history of severe anaphylactic reaction may be more reactive to repeated challenge.
Dosage & Administration
- The usual starting dose of Adorbis for adults is 5 mg once daily, to be individualized to the needs of the patient. If the response to 5 mg is inadequate, the dose may be increased to 10 mg and then to 20 mg once daily, with an appropriate interval for dose titration of 2 weeks. In patients with hepatic impairment or renal dysfunction, the initial daily dose should be 5 mg, and caution must be used in dose titration. The dose may not usually need adjustment in the elderly, unless there is significant renal or hepatic dysfunction.
Interaction
- Adorbis should not be combined with other β-blocking agents. Patients receiving catecholamine-depleting drugs or centrally active antihypertensive agents should be monitored closely when using Adorbis. Care should be taken when using Adorbis concurrently with antiarrhythmic agents and calcium channel blockers with negative inotropic effects, as it can lead to severe hypotension, bradycardia, and cardiac failure.
Contraindications
- Adorbis is contraindicated in patients with cardiogenic shock, overt heart failure, second or third degree A-V block, right ventricular failure secondary to pulmonary hypertension, and sinus bradycardia.
Side Effects
- Side effects associated with Adorbis may include fatigue, dizziness, headache, disturbances of the gut, cold or numb extremities, muscle weakness or cramps, and breathing difficulties.
Pregnancy & Lactation
- Bisoprolol fumarate should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts of bisoprolol fumarate have been detected in the milk of lactating rats, and it is not known whether this drug is excreted in human milk. If use of bisoprolol fumarate is considered essential, then mothers should consider stopping nursing.
Precautions & Warnings
- Caution is advised in adjusting the dose of Adorbis in patients with renal or hepatic impairment. Patients with a history of severe anaphylactic reaction to a variety of allergens may be more reactive to repeated challenge, and may be unresponsive to the usual doses of epinephrine used to treat allergic reactions.
