Description
Adorbis 10 mg Tablet is a beta-blocker medication used to lower blood pressure, reduce chest pain, and manage heart failure. It works by blocking adrenaline effects, slowing heart rate, and widening blood vessels.
Dosage
The usual starting dose for adults is 5 mg once daily, which may be increased to 10 mg and then to 20 mg. Pediatric use is not established. Patients with renal or hepatic impairment should use caution in dose adjustment. Elderly patients may require dose adjustment in the presence of significant renal or hepatic dysfunction.
Indications
- Hypertension
- Angina
- Moderate to severe heart failure
Mode of Action
Bisoprolol Hemifumarate selectively blocks ß1 adrenergic receptor, reduces heart rate, and decreases arterial hypertension. It does not affect cholesterol fraction including HDL-cholesterol. It has high bioavailability, balanced clearance, and an elimination half-life of 10-12 hours.
Interaction
- Other Beta-blocking Agents: Should not be combined with other β-blocking agents.
- Catecholamine-Depleting Drugs: Patients receiving these drugs should be monitored closely due to potential excessive reduction of sympathetic activity.
- Centrally Active Antihypertensive Agents: May exacerbate rebound hypertension which can follow clonidine withdrawal. Introduction of β-blockers should be delayed for several days after clonidine administration has stopped.
- Antiarrhythmic Agents: Should be used with care when myocardial depressants, inhibitors of A-V conduction, or certain calcium antagonists are used concurrently.
- Calcium Channel Blockers: Combined use can lead to prolongation of S-A and A-V conduction, especially in patients with impaired ventricular function or conduction abnormalities.
Contraindications
In patients with cardiogenic shock, overt heart failure, second or third degree A-V block, right ventricular failure secondary to pulmonary hypertension, and sinus bradycardia.
Side Effects
- Fatigue
- Dizziness
- Headache
- Gastrointestinal disturbances (nausea, vomiting, diarrhea, constipation, abdominal pain)
- Cold or numb extremities
- Muscle weakness or cramps
- Breathing difficulties (bronchospasm in asthma or COPD)
Pregnancy & Lactation
- Pregnancy: Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
- Lactation: If essential, nursing should be ceased as it is not known whether the drug is excreted in human milk.
Precautions & Warnings
- Renal or Hepatic Impairment: Use caution in adjusting the dose in patients with renal or hepatic impairment.
- Anaphylactic Reaction History: Patients with a history of severe anaphylactic reaction to allergens may be more reactive to repeated challenges and unresponsive to usual doses of epinephrine.
