Indications
- Local anaesthetic for infiltration blockade and intravenous regional analgesia
- Prevention and treatment of ventricular tachyarrhythmias
Description
- Sterile, non-pyrogenic, aqueous solution
- Contains Jasocaine, a local anaesthetic of the amide type
- Fast onset and intermediate duration of action
- Stabilizes the neuronal membrane
- Inhibits the ionic fluxes required for the initiation and conduction of impulses
Pharmacology
- Inhibits sodium influx through sodium specific ion channels
- Acts on voltage-gated sodium channels
- Binds more readily to sodium channels in activated state
- Causes state dependent blockade
Dosage
- Adjusted according to patient response and site of administration
- Maximum individual dose for healthy adults should not exceed 4.5mg/kg of body weight
- Maximum total dose for healthy adults should not exceed 300 mg
- Recommended doses for infiltration, intravenous regional, peripheral nerve, sympathetic nerve, central neural, and other blocks
Administration
- Intravenous dose of 1 mg.kg^-1 may be given over 2 min and repeated after 5 min
- Infusion of 2-4 mg.min^-1 to produce a plasma level of 2-4 mg.l^-1
- Loading dose of approximately 1 mg.kg^-1 given intravenously over 2 min at the outset
Interaction
- Cimetidine can impair the metabolism of lidocaine
- Increases in plasma lidocaine concentrations during concomitant therapy with beta blockers
- Prolongs the duration of action of suxamethonium
Contraindications
- Known hypersensitivity to anaesthetics of the amide type
Side Effects
- Adverse reactions to lidocaine are rare
- Result of excessively high blood concentrations
- Systemic effects involving the central nervous system and/or the cardiovascular system
- Excitatory and/or depressant reactions
- Cardiovascular reactions - hypotension, myocardial depression, bradycardia, cardiac arrest
- Allergic reactions are extremely rare
Pregnancy & Lactation
- Should not be given during early pregnancy unless benefits outweigh the risks
- Caution should be exercised when administered to a nursing woman
Precautions & Warnings
- Should be employed only by clinicians well versed in diagnosis and management of dose-related toxicity
- Depend on proper dosage, correct technique, adequate precautions, and readiness for emergencies
- Resuscitative equipment, oxygen and other resuscitative drugs should be available
- Used cautiously in patients with epilepsy, impaired cardiac conduction, bradycardia, impaired respiratory function, and impaired hepatic function
- Effect may be reduced if injected into an inflamed or infected area
- Repeated doses may cause significant increases in blood levels
- Careful and constant monitoring of cardiovascular and respiratory vital signs
Use in Special Populations
- Dosages in children should be reduced
- Elderly patients may be more sensitive to systemic effects and may require dose reductions
