Indications

• Local anaesthetic for infiltration blockade
• Intravenous regional analgesia
• Prevention of ventricular tachyarrhythmias

• Amide type local anaesthetic

• Inhibits ionic fluxes for impulse conduction

Pharmacology

• Inhibits sodium influx
• Voltage-gated sodium channels
• Faster onset in rapidly firing neurons
• State dependent blockade

Dosage

• Adjusted according to patient response and site of administration
• Maximum individual dose for healthy adults 4.5mg/kg
• Maximum total dose should not exceed 300 mg
• Specific recommended doses for various procedures and nerve blocks

Administration

• Intravenous dose for antiarrhythmic treatment
• Loading dose for achieving steady state

Interaction

• Cimetidine impairment of lidocaine metabolism
• Increased plasma lidocaine concentrations with beta blockers
• Prolonged duration of action with suxamethonium

Contraindications

• Known hypersensitivity to amide type anaesthetics
