Indications
- Treatment of essential hypertension in adults
- Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease or type 2 diabetes mellitus with target organ damage
Pharmacology
Telmisartan is a non-peptide angiotensin-ll receptor antagonist.
Dosage & Administration
- Usual starting dose of 40 mg once a day
- Blood pressure response is dose-related over the range of 20 to 80 mg
Pediatric use
Safety and effectiveness not established
Geriatric use
No overall differences in effectiveness and safety compared to younger patients
Hepatic impairment
Monitor carefully and up titrate slowly in patients with biliary obstructive disorders or hepatic insufficiency
Interaction
- Increased risk of renal impairment and loss of antihypertensive effect when co-administered with NSAIDS
- Avoid co-administration with aliskiren in patients with diabetes
Contraindications
Known hypersensitivity to this product or any of its components
Side Effects
- Common side effects in hypertensive patients: sinus pain and congestion, back pain, diarrhea
- Common side effects in cardiovascular risk reduction patients: intermittent claudication, skin ulcer
Pregnancy & Lactation
Category C (first trimester) and Category D (second and third trimester)
Precautions & Warnings
- Avoid fetal or neonatal exposure
- Monitor carefully in patients with impaired hepatic or renal function
- Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker
Overdose Effects
Manifestations may include hypotension, dizziness, tachycardia, bradycardia, increase in serum creatinine, and acute renal failure
