Description
- Treatment of essential hypertension in adults
- Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease or type 2 diabetes mellitus with documented target organ damage
Dosage & Administration
- Usual starting dose of 40mg once a day
- Blood pressure response is dose-related over the range of 20 to 80 mg
- Maximal reduction is generally attained after 4 weeks
- May be administered with or without food
- Not recommended for use in patients ≥75 years old or with hepatic impairment
Pharmacology
- Non-peptide angiotensin-ll receptor antagonist
- Blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin-ll
- Does not affect the response to bradykinin
- Does not bind to or block other hormone receptors or ion channels important in cardiovascular regulation
Interactions
- Increased risk of renal impairment and loss of antihypertensive effect with NSAIDS
- Co-administration with aliskiren in patients with diabetes should be avoided
Contraindications
- Known hypersensitivity to the product or any of its components
Side Effects
- Common side effects in hypertensive patients include sinus pain, back pain, and diarrhea
- Common side effects in cardiovascular risk reduction patients include intermittent claudication and skin ulcer
Pregnancy & Lactation
- Pregnancy Category C (first trimester) and D (second and third trimester)
- Consider potential adverse effects on the nursing infant when deciding whether to discontinue nursing or discontinue the drug
Precautions & Warnings
- Avoid fetal or neonatal exposure
- Monitor carefully in patients with impaired hepatic or renal function
- Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker
Overdose Effects
- Likely manifestations include hypotension, dizziness, tachycardia, increase in serum creatinine, and acute renal failure
