Symptomatic Relief

Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 reduces pain, inflammation, and fever with decreased potential for GI toxicity and effects on platelet aggregation
- Dose-dependent inhibition of COX-2 without inhibition of COX-1 at doses up to 150 mg daily
Dosage & Administration
- 30 mg once daily for Osteoarthritis
- 90 mg once daily for Rheumatoid arthritis and Ankylosing spondylitis
- 120 mg once daily for Acute gouty arthritis
- 90 mg once daily for Postoperative dental surgery pain, limited to a maximum of 3 days
- Patients may require additional postoperative analgesia
- Shortest duration and lowest effective daily dose should be used to minimize cardiovascular risks
- Periodic re-evaluation of patient's need for relief and response to therapy
Medicine Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduced effect of diuretics and antihypertensive drugs
- Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Possible increase in nephrotoxic effect of ciclosporin or tacrolimus
- Decreased lithium renal excretion leading to increased lithium plasma levels
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or GI bleeding
- History of bronchospasm, acute rhinitis, nasal polyps, allergic-type reactions to acetylsalicylic acid or NSAIDs including COX-2 inhibitors
- Pregnancy and lactation
- Severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure (NYHA ll-IV)
- Uncontrolled hypertension, established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Not recommended for use during breastfeeding
Precautions & Warnings
- Caution with patients at risk of GI complications, elderly, concurrent use of NSAIDs or acetylsalicylic acid, or prior history of gastrointestinal disease
- Careful consideration for patients with significant risk factors for cardiovascular events
- Potential reduction in prostaglandin formation and renal blood flow leading to impaired renal function
- Caution in patients with history of cardiac failure, left ventricular dysfunction, hypertension, or pre-existing edema
- Monitoring of renal and liver function, discontinuation at first sign of hypersensitivity, caution with warfarin or other anticoagulants
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
- Usual supportive measures in case of overdose
