Indications
- Osteoarthritis (OA)
- Rheumatoid arthritis (RA)
- Ankylosing spondylitis
- Acute gouty arthritis
- Moderate pain associated with dental surgery
Symptomatic Relief Dosage
- 30 mg once daily for OA
- 90 mg once daily for RA and Ankylosing spondylitis
- 120 mg once daily for acute gouty arthritis
- 90 mg once daily for postoperative dental surgery pain
Usage
- Limited to a maximum of 3 days for postoperative dental surgery pain
- Shortest duration possible and lowest effective daily dose should be used
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Decreases clinical signs and symptoms with decreased potential for gastrointestinal (GI) toxicity
- Produces dose-dependent inhibition of COX-2 without inhibition of COX-1 at doses up to 150 mg daily
Interactions
- Increase in prothrombin time with oral anticoagulants
- Reduction of the effect of diuretics and other antihypertensive drugs with NSAIDs
- Safe concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
Side-effects
- Palpitation
- Fatigue
- Influenza-like symptoms
- Dry mouth
- Taste disturbance
- Anxiety
- Myalgia and arthralgia
- Confusion and hallucinations (very rarely)
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Excreted in the milk of lactating rats
- Must not breastfeed
Precautions & Warnings
- Caution with patients at risk of gastrointestinal complications with NSAIDs
- Consideration for patients with significant risk factors for cardiovascular events
- Monitoring of renal function in patients with reduced prostaglandin formation
- Caution in patients with symptoms and/or signs suggesting liver dysfunction
- Discontinue at the first appearance of skin rash or hypersensitivity
Overdose Effects
- Single doses up to 500 mg and multiple doses up to 150 mg/day did not result in significant toxicity
