  • Exerts direct inhibitory effect on the parasympathetic nervous system

Pharmacology

  • Non-selective muscarinic acetylcholine receptor antagonist
  • Binds with higher affinity to M1 subtype
  • Higher affinity for central muscarinic receptors in cerebral cortex
  • Lower affinity for peripherally located central muscarinic receptors

Dosage & Administration

  • Individualized dosage
  • Low initial dose, gradually increased
  • Dosing determined by patient's reaction
  • Postencephalitic patients may require atropine adjuvant

Interaction

  • Additive effects with cannabinoids, barbiturates, opiates, and alcohol
  • Increased sedative effects with concurrent use of alcohol or other CNS depressants
  • Monoamine oxidase inhibitors and tricyclic antidepressants intensify anticholinergic effects
  • Increased risk of tardive dyskinesia during concomitant administration of anticholinergics and neuroleptics

Contraindications

  • Hypersensitivity to Trihexyphenidyl
  • Narrow angle glaucoma
  • Blindness reported due to long-term use in narrow angle glaucoma

Pregnancy & Lactation

  • Limited data on use during pregnancy
