- Exerts direct inhibitory effect upon the parasympathetic nervous system
- Relaxing effect on smooth musculature
Pharmacology
- Binds with higher affinity to the M1 subtype of muscarinic acetylcholine receptors
- Modifies nicotinic acetylcholine receptor neurotransmission
- Enhances dopamine release in the striatum
- Mechanism of action not fully elucidated
Dosage & Administration
- Individualized dosage
- Low initial dose with gradual increase
- Dose adjustment in concomitant use with levodopa or other parasympathetic inhibitors
- Best tolerated if divided into 3 doses
Interaction
- Additive effects with cannabinoids, barbiturates, opiates, and alcohol
- Increased sedative effects with alcohol or other CNS depressants
- Intensified anticholinergic effects with monoamine oxidase inhibitors and tricyclic antidepressants
- Increased risk for tardive dyskinesia during concomitant administration with neuroleptics

- Hypersensitivity to Trihexyphenidyl
- Narrow angle glaucoma

- Exacerbation of parkinsonism with abrupt treatment withdrawal
- Neuroleptic malignant syndrome with abrupt treatment withdrawal
Pregnancy & Lactation
- Not recommended during pregnancy unless need clearly established
- Caution advised during lactation due to potential suppression of lactation
Precautions & Warnings
- Gonioscope evaluation prior to initiation of therapy
- Close monitoring of intraocular pressures
- Caution in hot weather and concomitant use with other atropine-like drugs
- Potential occurrence of anhidrosis and hyperthermia
- Potential occurrence of Neuroleptic Malignant Syndrome (NMS)
Overdose Effects
- Central symptoms of atropine intoxication
- Neuropsychiatric signs
- Hyperpyrexia, coma, cardiac and respiratory arrest
- Gastric lavage or methods to limit absorption recommended for acute overdose
