Dosage

  • Furosemide 20 and spironolactone 50 mg
  • Furosemide 40 and spironolactone 50 mg
  • Adult Dosage: 1 to 4 tablets daily
  • Use in children: Not suitable for use in children
  • Elderly Patients: May be excreted more slowly in the elderly

Mode of Action

  • Complementary mechanisms of spironolactone and furosemide
  • Additive or synergistic diuretic effects
  • Inhibits Na+/K+/2Cl- co-transporter
  • Inhibits reabsorption of sodium and potassium
  • Reduced potassium loss induced by furosemide

Indications

  • Essential hypertension
  • Chronic congestive heart failure
  • Hepatic cirrhosis with ascites
  • Swelling due to excess fluid retention (edema)
  • Hyperaldosteronism
  • Resistant edema with secondary hyperaldosteronism

Interaction

  • Increased risk of hyperkalemia with ACE inhibitors or potassium salts
  • Increased levels of cardiac glycosides like digoxin
  • Possibility of digitalis toxicity
  • Potential hypokalemia with corticosteroids
  • Reduced diuretic effects when used with indomethacin or NSAIDs
  • Increased ototoxicity with aminoglycoside antibiotics
  • Reduction in natriuretic and anti-hypertensive effect with sucralfate

Contraindications

  • Anuria
  • Acute renal insufficiency
  • Severe impairment of renal function
  • Hyperkalemia
  • Addison's disease
  • Hypersensitivity to spironolactone, furosemide, or sulphonamides

Pregnancy & Lactation

  • Spironolactone and its metabolites may cross the placental barrier
  • Use of furosemide in women of childbearing age should consider potential risks to the fetus
  • Spironolactone metabolites detected in breast milk
  • Discontinue breastfeeding if essential to use furosemide

  • Caution in patients liable to electrolyte deficiency
  • Use with caution in diabetes
  • Use with caution in enlarged prostate
  • Use with caution in hypotension
