Indications
- Treatment of Acute and chronic psychoses
- Schizophrenia
- Bipolar disorder
- Manic episodes

Pharmacology
- Chemical class: dibenzothiazepine derivatives
- Antagonist at multiple neurotransmitter receptors
- No appreciable affinity at cholinergic muscarinic and benzodiazepine receptors
- Mechanism of action is unknown
- Efficacy mediated through dopamine D2 and serotonin 5HT2 antagonism
Dosage & Administration
- Administered twice daily
- First 4 days dosage titration
- Usual effective dose range of 300-450 mg/day
- Maintenance treatment in combination with a mood stabilizer
- Dosage adjustments for manic and depressive episodes
- 300 mg/day dosage for depressive episodes
Interaction
- Caution with medicines causing electrolyte imbalance
- Co-administration with thioridazine or carbamazepine
- Increases in the clearance of Tiapine XR with concomitant use of certain medications
Contraindications
- Hypersensitivity to Quetiapine

- Elevations in serum triglyceride levels
- Weight gain
- Extrapyramidal symptoms
Pregnancy & Lactation
- Use only if benefits justify potential risks during pregnancy
- Avoid breastfeeding while taking Quetiapine
Precautions & Warnings
- Risk of suicidal thoughts and self-harm
- Caution in patients with cardiovascular disease
- Increased risk of orthostatic hypotension
- Caution in patients with history of seizures
- Consideration of dose reduction if tardive dyskinesia appears
- Discontinuation and appropriate medical treatment for Neuroleptic Malignant Syndrome
- Exercise caution in patients with cardiovascular disease or family history of QT prolongation
- Monitoring for neutropenia and hyperglycemia
Use in Special Populations
- Caution in the elderly due to reduced plasma clearance
- Not indicated for use in children and adolescents below 18 years of age
Overdose Effects
- Survival reported in acute overdoses of up to 30 grams
- No specific antidote
- Intensive care for severe intoxication
- Treatment for refractory hypotension
- Close medical supervision and monitoring
