Indications

Pharmacology

  • Novel bronchodilator
  • Selective inhibition of phosphodiesterase 4
  • Relaxation of bronchial smooth muscle
  • Inhibition of platelet activating factor (PAF)
  • Inhibition of leukotriene production

Dosage & Administration

  • Elderly: 200 mg tablet two or three times daily
  • Adults: 400 mg tablet two or three times daily
  • Children >12 years of age: 10 ml syrup or 200 mg tablet two or three times daily
  • Children 6-12 years of age: 6-9 mg/kg body weight two times daily
  • Doxophylline SR tablet to be taken once daily if required daily dose is 400 mg

Interaction

  • Avoid administration with other xanthine preparations
  • Limit consumption of caffeine-containing beverages and food
  • Caution with concurrent administration of ephedrine or sympathomimetic drugs
  • Potential interaction with erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, propanolol, phenytoin, and other anticonvulsants

Contraindications

  • Known hypersensitivity to doxophylline or other xanthine derivatives
  • Acute myocardial infarction
  • Hypotension
  • Lactation

Pregnancy & Lactation

  • Use during pregnancy should be evaluated carefully on a case-by-case basis
  • Contraindicated during lactation
  • Effects on ability to drive and use machines: Does not affect alertness

Precautions & Warnings

  • Factors such as age, cardiac disease, liver disease, concomitant infections, and certain drug interactions may affect hepatic clearance
  • Caution in patients with cardiac disease, hypertension, hypoxemia, hyperthyroidism, chronic corpulmonale, congestive heart failure, liver disease, peptic ulcer, and renal impairment
  • Monitoring of blood levels recommended in cases affecting clearance

Overdose Effects

  • Severe cardiac arrhythmias
  • Tonic-clonic seizure
  • Discontinuation of treatment may be necessary
  • Symptomatic treatment of cardiovascular collapse may be required
