Indications
- Treatment of major depressive disorder (MDD)
Pharmacology
- Enhancement of central noradrenergic and serotonergic activity
- Antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
- Potent antagonist of 5- HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
- Moderate peripheral α1-adrenergic antagonist
- Moderate antagonist at muscarinic receptors
- Potent antagonist of histamine (H<sub>1</sub>) receptors
- Metabolized and eliminated via urine and faeces
Dosage & Administration
- Recommended starting dose of 15 mg/day
- Dose range of 15 to 45 mg/day
- Elimination half-life of approximately 20 to 40 hours
- Use in children not recommended
- Instructions for missed dose
Interaction
- Clinically significant drug-drug interactions with Monoamine Oxidase Inhibitors (MAOI) and other serotonergic drugs
- Possible interruption of the metabolism or activity of Carbamazepine, Phenytoin or Cimetidine
- Avoidance of Alcohol & Diazepam
Contraindications
- Contradicted in patients with known hypersensitivity to Mirtazapine or excipients
- Concomitant use with monoamine oxidase (MAO) inhibitor
Side Effects
- Common side effects including dizziness, drowsiness, dry mouth, increased appetite, and weight gain
Pregnancy & Lactation
- Pregnancy Category-C
- Advice for notifying physician if becoming pregnant or breastfeeding during therapy
Precautions & Warnings
- Alertness to emergence of certain behaviors and worsening of depression
- Warning about risk of developing agranulocytosis
- Potential impairment of judgment, thinking, and motor skills
- Possibility of clinically significant ALT (SGPT) elevations
