Indications
- Treatment of ALK-positive metastatic NSCLC
- Patients intolerant to Crizotinib
Pharmacology
- Inhibitor of multiple kinases including ALK, ROS1, IGF-1R, and FLT-3
- Inhibits autophosphorylation of ALK
- Metabolized by CYP2C8 and CYP3A4
Dosage & Administration
- Initial dose: 90 mg orally once daily for 7 days
- Increased to 180 mg orally once daily if tolerated
- May be taken with or without food
- Tablets should be swallowed whole
Interaction
- Avoid concomitant use with strong CYP3A inhibitors
- Use caution with strong CYP3A inducers
- May alter plasma concentrations of CYP3A substrates
Contraindications
- Known hypersensitivity to Brigatinib or components
Side Effects
- Interstitial Lung Disease (ILD)/Pneumonitis
- Hypertension
- Bradycardia
- Visual Disturbance
- CPK Elevation
- Pancreatic Enzyme Elevation
- Hyperglycemia
Pregnancy & Lactation
- No clinical data on use in pregnant women
- Lactating women advised not to breastfeed
- Use effective non-hormonal contraception during treatment
Precautions & Warnings
- Severe pulmonary adverse reactions have occurred
- Hypertension should be controlled prior to treatment
- Monitor heart rate and blood pressure
- Patients should report visual symptoms
- Monitor CPK, amylase, and lipase levels
- Assess fasting serum glucose levels
- Store below 30°C, away from light & moisture
