Indications

  • Treatment of anemia in Chronic Kidney Disease (CKD) patients
  • Treatment of anemia in Zidovudine-treated HIV-infected patients
  • Reduction of allogeneic RBC transfusions in noncardiac, nonvascular surgery patients
  • Prevention of anemia of prematurity in infants

Description

Recombinant human erythropoietin (EPO) expressed in Chinese hamster ovary cells with a 165 amino acid sequence identical to human urinary EPO

Dosage

  • Evaluation of iron stores and nutritional factors before treatment
  • Monitoring hemoglobin levels weekly or monthly
  • Dosing adjustments based on hemoglobin rate of rise and decline
  • Different starting doses and dose adjustments for CKD patients on dialysis and not on dialysis, Zidovudine-treated HIV-infected patients, and patients on cancer chemotherapy

Administration

  • Do not shake the product
  • Protect syringe from light
  • Inspect visually for particulate matter and discoloration before administration
  • Discard unused portions of the product

Interaction

Potentiation of Hemocrit effect by simultaneous therapeutic administration of a haematinic agent such as ferrous sulphate

Contraindications

  • Uncontrolled hypertension
  • Serious allergic reactions to Erythropoietin alfa
  • Patients who develop Pure Red Cell Aplasia (PRCA) following treatment with any erythropoietin

Side Effects

  • Adverse reactions in CKD patients, Zidovudine-treated HIV-infected patients, cancer patients on chemotherapy, surgery patients, and premature infants

Pregnancy & Lactation

  • Use in pregnancy only if benefit outweighs risk to the fetus
  • Not recommended for pregnant or lactating surgical patients participating in an autologous blood predonation program
  • Caution advised in nursing women

Overdose Effects

  • Hemoglobin levels above desired level
  • Management with discontinuation or reduction of dosage and/or phlebotomy
  • Cases of severe hypertension observed following overdose
