Indications
- Symptomatic relief of osteoarthritis (OA)
- Symptomatic relief of rheumatoid arthritis (RA)
- Symptomatic relief of ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Decreases clinical signs and symptoms with decreased potential for gastrointestinal (GI) toxicity
- Primarily responsible for the synthesis of prostanoid mediators of pain, inflammation, and fever
Dosage & Administration
- 30 mg once daily for osteoarthritis
- 60 mg once daily for osteoarthritis if insufficient relief
- 90 mg once daily for rheumatoid arthritis and ankylosing spondylitis
- 120 mg once daily for acute gouty arthritis for 8 days
- 90 mg once daily for postoperative dental surgery pain, limited to a maximum of 3 days
Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduction in the effect of diuretics and other antihypertensive drugs with NSAIDs
- Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Potential increase in nephrotoxic effect of ciclosporin or tacrolimus
- Decrease in lithium renal excretion and increase in lithium plasma levels with NSAIDs
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- Adverse reactions after taking acetylsalicylic acid or NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure, hypertension, established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Excreted in the milk of lactating rats
Precautions & Warnings
- Caution with patients at risk of developing a gastrointestinal complication with NSAIDs
- Caution with patients at significant risk factors for cardiovascular events
- Monitoring of renal function in patients with decreased renal blood flow
- Caution with patients with a history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of hepatic function and discontinuation upon signs of hepatic insufficiency
- Discontinuation at the first appearance of hypersensitivity signs
- Masking of fever and other signs of inflammation
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- No significant toxicity observed up to 500 mg as single doses and 150 mg/day for 21 days
